What is a sweetener and how does it taste sweet?

The taste cells on your tongue carry receptors that detect sweetness. When sugar binds to them, the brain receives a 'sweet' signal. Sweetener molecules bind to the same receptors, but because their structure is different the body cannot turn most of them into energy, or converts very little. The result is taste with almost no calories. That is the whole mechanism: not magic, just a different molecule fitting the same lock.

What does 'safe' rest on? The ADI concept

When regulators approve a sweetener they set an acceptable daily intake (ADI). The method is this: the highest dose showing no adverse effect in animal studies is identified, then that dose is usually divided by 100. This hundred-fold margin covers differences between species and differences in sensitivity between people. So the ADI is not a line where 'exceeding it harms you'; it is a threshold meaning 'even taking this much every day for life leaves a wide safety margin'.

Numbers make this concrete. The ADI for aspartame is about 40 milligrams per kilogram of body weight per day. For a 70-kilogram adult that works out at roughly 2,800 milligrams a day, far above the amount in a typical can of diet drink. In practice, reaching that limit with normal consumption is difficult. This does not mean sweeteners are 'unlimited good'; it simply shows what the safety assessment rests on.

The aspartame debate: what exactly happened in 2023?

In 2023 IARC, part of the World Health Organization, placed aspartame in group 2B, meaning 'possibly carcinogenic to humans'. In the same period JECFA, the joint WHO and FAO committee on additives, left the daily intake limit unchanged and continued to regard consumption within that limit as safe. In the headlines these two were usually mixed up.

The confusion arises because the two bodies answer different questions. IARC asks 'could this substance be linked to cancer under some conditions', that is, it classifies hazard. JECFA asks 'how much risk exists at real consumption levels'. Group 2B also contains substances such as aloe vera extract and pickled vegetables; the group reflects the strength of the evidence, not the size of the risk. Why do sugar alcohols cause digestive complaints?

Sugar alcohols are only partly absorbed in the small intestine. The unabsorbed portion passes into the large intestine, draws water in and is fermented by gut bacteria. The gas produced and the extra water cause bloating, wind and diarrhoea in some people. This is neither an allergy nor poisoning; it is a purely mechanical and microbial process, and the threshold varies a great deal from person to person.

Under European Union rules, if the polyol content of a product passes a certain level the label must carry the warning 'excessive consumption may produce laxative effects'. You will see this often on sugar-free gum, sugar-free chocolate and products aimed at people with diabetes. Why is erythritol discussed separately?

Erythritol behaves differently from other sugar alcohols: most of it is absorbed in the small intestine and excreted unchanged in urine, so it is less likely to cause digestive complaints. In 2023 an observational study reported an association between high blood erythritol levels and cardiovascular events, which brought the topic into the news. Care is needed here: an observational association is not proof of cause and effect, and the body produces its own erythritol anyway. Do they help with weight loss? What the evidence says

Replacing a sugary drink with a sweetened one lowers the calories taken in over the short term and shows a measurable effect in studies. The long-term outcome, however, depends on whether that change fits into the overall diet. In 2023 the World Health Organization published guidance stating that it does not recommend sweeteners as a long-term solution for weight control on their own; that is not a ban, it means 'not a strategy by itself'.

The most common trap in practice is compensation: eating more elsewhere because 'I chose the sugar-free one'. Some animal studies and small human studies have reported findings suggesting that certain sweeteners can alter the composition of the microbiota. However, some of that work used doses far above real consumption, and the results are not consistent. The honest summary today is this: it is an open field of research, no firm verdict has been reached, and regulators have not changed the existing limits on these grounds.

The practical conclusion is not to avoid sweeteners but to see that focusing on a single component does not help. The best-evidenced factor for gut health is fibre intake, and that is where most people fall short. When discussing the sweetener in a heavily processed product, the fact that the same product contains no fibre is usually the more decisive one.

Sugar or sweetener? Framing the comparison properly

This question is usually framed the wrong way. When someone asks 'which is better, sweetener or sugar', the answer depends on what you are comparing it with. As an alternative to a sugary drink, a sweetened drink is better for calories and for teeth. But if water is an alternative to both, the choice is clearly water. A sweetener is a transitional tool on the way away from sugar; it is not the destination.

The same logic applies to 'are natural sweeteners better'. Stevia comes from a plant, but the steviol glycosides used in food are still purified, processed substances; being natural confers no automatic advantage. Honey and agave syrup, natural as they are, remain sugar as far as the body is concerned and count as free sugars. The word 'natural' on a label carries less information than the number in the nutrition table.
